In the square is the important basilica of Santa Maria in Cosmedin, which arose as a diaconia in the 7th century and is currently administered by monks of the Orthodox rite (in particular the Greek-Melchite “sui iuris” rite). Inside the basilica you can still see the colonnades of the Statio Annonae, or the portico where free distributions of grain to the plebs took place in the Republican era.

The mysterious and disturbing mask represents the face of a screaming faun, and originally it could have been a manhole cover of the Cloaca Maxima that runs right here underground, or perhaps part of the well of the temple of Mercury, which stood here in the archaic age, where merchants came to swear or to purify themselves of their false oaths during negotiations. Perhaps from this ancient purifying use comes the legendary use that, in fact, was made of it in the Middle Ages.

Each side is decorated with two rows of niches with a shell-shaped apse, which originally housed statues. The tops of the arches are decorated with full-round figures of Rome and Juno, Minerva and Ceres.
